HEALTH FORM

A current State of Connecticut Health form is required to be on file in the Nurse's Office before a student may Participate in any athletic activity. 

Physical Exams:
Physical exams must still be submitted to the McGee Middle School Nurse's office prior to any participation.  A physical exam must be current (within the past 13 months) and signed by the physician.
