District Health Services

7TH GRADE PHYSICAL AND IMMUNIZATION REQUIREMENT

The State of Connecticut requires that incoming 7th grade students have a physical exam on file with the school nurse. This is an opportunity to update the school about your child’s health status and provide required proof of immunizations. The following vaccines are required for entry into Grade 7: Tdap and meningococcal.  These are required BEFORE entry to 7th grade.

ELEVATOR USE

Students who need to use the elevator due to an injury or surgery need to report to the nurse upon arriving at school to obtain an elevator pass. A doctor’s note stating any special accommodations/restrictions needed should accompany the student as well. Elevator passes are given for a week's time. Students should report to the nurse weekly for an updated elevator pass and present any updated medical notes at that time. The nurse will also go over evacuation procedures for students currently using crutches/wheelchairs. Wheelchairs and crutches needed for a student during the school day must be provided by the student’s parent/guardian. The nurse will notify school counseling and teachers as needed of any accommodations needed for the school day.
